• A survey for program participants
• A survey for staff and volunteers
  

How to complete your survey online

To input a survey online, please select the appropriate survey button below. You will be presented with a list of participating organizations arranged alphabetically first by province and territory, then by city and town, and then by program name. Please select your centre and input your survey. When you have finished, click the Submit button. If you cannot locate your program's name in the list, ask one of the staff members at the program whether it is participating. For more information, see Frequently Asked Questions.
